目的 探討散結(jié)鎮(zhèn)痛膠囊聯(lián)合屈螺酮炔雌醇片治療子宮內(nèi)膜異位癥的臨床療效。方法 選取2019年8月-2022年9月在聊城市第二人民醫(yī)院(山東第一醫(yī)科大學附屬聊城二院)就診的150例子宮內(nèi)膜異位癥患者,按照隨機數(shù)字表法將150例患者分為對照組和治療組,每組各75例。對照組于月經(jīng)來潮第5天口服屈螺酮炔雌醇片,1次/d,1片/次,直至下一個月經(jīng)周期。治療組在對照組基礎上口服散結(jié)鎮(zhèn)痛膠囊,3次/d,4粒/次。兩組連續(xù)治療6個月經(jīng)周期。觀察兩組的臨床療效,比較兩組的主要癥狀消退情況、病灶最大直徑、子宮體積、白細胞介素-8(IL-8)、血管內(nèi)皮生長因子(VEGF)、中性粒細胞淋巴細胞比值(NLR)。結(jié)果 治療后,治療組的總有效率(93.33%)比對照組總有效率(82.67%)高,差異有統(tǒng)計學意義(P<0.05)。治療后,治療組患者痛經(jīng)、盆腔壓痛、肛門墜痛消失時間明顯短于對照組,差異有統(tǒng)計學意義(P<0.05)。治療后,兩組的病灶最大直徑、子宮體積均顯著減少(P<0.05);治療后,治療組的病灶最大直徑、子宮體積低于對照組,差異有統(tǒng)計學意義(P<0.05)。治療后,兩組的血清IL-8、VEGF、NLR明顯降低(P<0.05);治療后,治療組的血清IL-8、VEGF、NLR低于對照組,差異有統(tǒng)計學意義(P<0.05)。結(jié)論 散結(jié)鎮(zhèn)痛膠囊聯(lián)合屈螺酮炔雌醇片治療子宮內(nèi)膜異位癥的療效確切,能改善臨床癥狀,縮小病灶和子宮大小,降低炎癥反應,安全性較好。

Objective To investigate the clinical effect of Sanjie Zhentong Capsules combined with Drospirenone and Ethinylestradiol Tablets in treatment of endometriosis. Methods Patients (150 cases) with endometriosis in the Second People's Hospital of Liaocheng (Liaocheng Second Hospital Affiliated to Shandong First Medical University) from August 2019 to September 2022 were divided into control and treatment groups according to the random number table method, and each group had 75 cases. Patients in the control group were po administered with Drospirenone and Ethinylestradiol Tablets on the 5th day of menstruation, 1 tablet/time, once daily, until the next menstrual cycle. Patients in the treatment group were po administered with Sanjie Zhentong Capsules on the basis of the control group, 4 grains/time, three times daily. Patients in two groups were treated for 6 months. After treatment, the clinical efficacies were evaluated, and the symptom disappearance time, maximum diameter of the lesion, uterine volume, IL-8, VEGF, and NLR in two groups were compared. Results After treatment, the total effective rate of the treatment group (93.33%) was higher than that of the control group (82.67%), with a statistically significant difference (P < 0.05). After treatment, the disappearance time of dysmenorrhea, pelvic tenderness, and anal sagging pain in the treatment group was significantly shorter than that in the control group, with a statistically significant difference (P < 0.05). After treatment, the maximum diameter of the lesion and uterine volume in two groups were significantly reduced (P < 0.05). After treatment, the maximum diameter of the lesion and uterine volume in the treatment group were lower than those in the control group, with statistically significant differences (P < 0.05). After treatment, the IL-8, VEGF, and NLR in two groups were significantly reduced (P < 0.05). After treatment, the IL-8, VEGF, and NLR in the treatment group were significantly lower than those in the control group (P < 0.05). Conclusion Sanjie Zhentong Capsules combined with Drospirenone and Ethinylestradiol Tablets is effective in the treatment of endometriosis, which can improve clinical symptoms, reduce the size of lesions and uterus, reduce inflammatory reactions, and has good safety.
