目的 探討復(fù)方甘菊利多卡因凝膠聯(lián)合聚維酮碘含漱液治療牙周炎的臨床療效。方法 選擇2022年2月—2023年2月在秦皇島市海港醫(yī)院治療的66例牙周炎患者為研究對象，依據(jù)用藥方案的差別分為對照組（33例）和治療組（33例）。對照組含漱聚維酮碘含漱液，10 mL/次，每次10秒，4次/d；在此基礎(chǔ)上，治療組給予復(fù)方甘菊利多卡因凝膠，每次涂0.5 cm凝膠于牙齦處并稍加按摩，3次/d。兩組患者均經(jīng)2周治療。觀察兩組患者臨床療效，比較治療前后兩組患者癥狀改善時(shí)間，GSES評分、OHIP-14評分和VAS評分，牙周指標(biāo)探診深度（PD）、齦溝出血指數(shù)（SBI）、臨床附著水平（CAL）和菌斑指數(shù)（PLI）水平，及血清學(xué)指標(biāo)C反應(yīng)蛋白（CRP）、白細(xì)胞介素-1β（IL-1β）、白細(xì)胞介素-17（IL-17）、高遷移率族蛋白B1（HMGB1）、白細(xì)胞介素-6（IL-6）和降鈣素基因相關(guān)肽（CGRP）水平。結(jié)果 治療后，治療組臨床有效率（96.97%）明顯高于對照組（75.76%，P＜0.05）。治療后，治療組癥狀改善時(shí)間均明顯早于對照組（P＜0.05）。治療后，兩組GSES評分明顯升高，而OHIP-14評分和VAS評分均明顯降低（P＜0.05），并以治療組量表評分改善最為顯著（P＜0.05）。治療后，兩組患者PD、SBI、CAL、PLI、CRP、IL-1β、IL-17、HMGB1和IL-6水平均明顯下降，而CGRP水平明顯升高（P＜0.05），且治療組患者上述指標(biāo)改善最顯著（P＜0.05）。結(jié)論 復(fù)方甘菊利多卡因凝膠聯(lián)合聚維酮碘含漱液治療牙周炎可有效改善臨床癥狀，降低機(jī)體炎癥反應(yīng)，促進(jìn)患者口腔健康與自我效能感。

Objective To investigate the clinical efficacy of Compound Chamomile and Lidocaine Hydrochloride Gel combined with Povidone-iodine Gargle and Mouthwash in treatment of periodontitis. Methods Patients (66 cases) with periodontitis in Qinhuangdao Haigang Hospital from February 2022 to February 2023 were divided into control (33 cases) and treatment (33 cases) group based on different treatments. Patients in the control group were administered with Povidone-iodine Gargle and Mouthwash, 10 mL/time, 10 s/time, four times daily. Patients in the treatment group were administered with Compound Chamomile and Lidocaine Hydrochloride Gel on the basis of the control group, 0.5 cm gel Applied to the gums each time and massage slightly, three times daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical evaluation was evaluated, and the improvement time of symptom, the scores GSES, OHIP-14 and VAS, the levels of periodontal indicators PD, SBI, CAL and PLI, the levels of serological indexes CRP, IL-1β, IL-17, HMGB1, IL-6 and CGRP in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group (96.97%) was significantly higher than that of the control group (75.76%, P < 0.05). After treatment, the improvement time of symptom in the treatment group was significantly earlier than that in the control group (P < 0.05). After treatment, the GSES score was significantly increased, while the OHIP-14 score and VAS score were significantly decreased in two groups (P < 0.05), and the score of the treatment group was improved most significantly (P < 0.05). After treatment, the levels of PD, SBI, CAL, PLI, CRP, IL-1β, IL-17, HMGB-1 and IL-6 were significantly decreased, while the level of CGRP was significantly increased in two groups (P < 0.05), and the above-mentioned indexes were improved most significantly in the treatment group (P < 0.05). Conclusion Compound Chamomile and Lidocaine Hydrochloride Gel combined with Povidone-iodine Gargle and Mouthwash in treatment of periodontitis can effectively improve clinical symptoms, reduce inflammatory reaction, and promote patients’ oral health and self-efficacy.
