目的 觀察疏風(fēng)解毒膠囊聯(lián)合孟魯司特鈉和丙酸氟替卡松治療兒童痰熱郁結(jié)型腺樣體肥大的臨床療效。方法 選取2022年5月—2023年3月安徽中醫(yī)藥大學(xué)第一附屬醫(yī)院收治的70例痰熱郁結(jié)型腺樣體肥大的患兒，隨機(jī)分為對(duì)照組（35例）和治療組（35例）。對(duì)照組患兒口服孟魯司特鈉顆粒，4 mg/次，1次/d，以及噴鼻丙酸氟替卡松鼻噴霧劑，每側(cè)鼻孔50μg/次，2次/d。治療組患兒在上述方案的基礎(chǔ)上口服疏風(fēng)解毒膠囊，0.52 g/次，3次/d。兩組患兒均治療6周。觀察兩組患兒臨床療效，比較治療前后兩組患兒中醫(yī)證候主癥和次癥單項(xiàng)積分，腺樣體厚度/鼻咽腔寬度比值（A/N）積分，OSA-18積分和復(fù)發(fā)率。結(jié)果 治療后，治療組臨床有效率為93.94%，明顯高于對(duì)照組（75.00%，P＜0.05）。治療后，兩組中醫(yī)證候主癥和次癥單項(xiàng)積分，及A/N值和OSA-18積分均低于本組治療前（P＜0.05），且治療組各項(xiàng)積分下調(diào)水平更加顯著（P＜0.05）。治療1個(gè)月后，治療組復(fù)發(fā)率明顯低于對(duì)照組（3.23% vs 20.83%，P＜0.05）。結(jié)論 疏風(fēng)解毒膠囊聯(lián)合孟魯司特鈉和丙酸氟替卡松治療兒童痰熱郁結(jié)型腺樣體肥大療效確切，可有效緩解癥狀，縮小腺樣體指數(shù)，提高生活質(zhì)量，且安全性好，預(yù)后近期復(fù)發(fā)率較低。

Objective To observe the clinical efficacy of Shufeng Jiedu Capsules combined with montelukast sodium and fluticasone propionate in treatment of adenoid hypertrophy of phlegm-heat stagnation in children. Methods Children (70 cases) with adenoid hypertrophy of phlegm-heat in the First Affiliated Hospital of Anhui University of Chinese Medicine from May 2022 to March 2023 were randomly divided into control (35 cases) and treatment (35 cases) group. Children in the control group were po administered with Montelukast Sodium Oral Granules, 4 mg/time, once daily, and they were administered with Fluticasone Propionate Nasal Spray, 50 μg/time for each nostril, twice daily. Children in the treatment group were po administered with Shufeng Jiedu Capsules on the basis of the control group, 0.52 g/time, three times daily. Children in two groups were treated for 6 weeks. After treatment, the clinical evaluation was evaluated, and the scores of TCM primary and secondary symptoms, scores of A/N value and OSA-18, and recurrence rate in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group was 93.94%, which was significantly higher than that of the control group (75.00%, P < 0.05). After treatment, the scores of main syndrome and secondary syndrome, A/N value and OSA-18 score in two groups were lower than those before treatment (P < 0.05), and the down-regulation level of each score in the treatment group was more significant (P < 0.05). After one month of treatment, The recurrence rate in the treatment group was significantly lower than that in the control group (3.23% vs 20.83%, P < 0.05). Conclusion Shufeng Jiedu Capsules combined with montelukast sodium and fluticasone propionate has a definite curative effect in treatment of adenoid hypertrophy of phlegm-heat stagnation in children, which can effectively relieve the symptoms, reduce the adenoid index, improve the quality of life, and the safety is good, and the short-term recurrence rate is low.
