目的 探討全天麻膠囊聯(lián)合尼莫地平片治療偏頭痛的臨床療效。方法 選取2021年2月—2023年4月上海市松江區(qū)泗涇醫(yī)院收治的88例偏頭痛患者，根據(jù)隨機(jī)數(shù)字表法將所有患者分為對(duì)照組和治療組，各44例。對(duì)照組口服尼莫地平片，2片/次，3次/d。治療組在對(duì)照組基礎(chǔ)上口服全天麻膠囊，6粒/次，3次/d。兩組連續(xù)治療12周。比較兩組的治療效果、偏頭痛癥狀、腦部血流動(dòng)力學(xué)指標(biāo)和血清指標(biāo)。結(jié)果 治療后，治療組的總有效率比對(duì)照組高，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。治療后，兩組的發(fā)作頻率、每次持續(xù)時(shí)間、VAS評(píng)分低于治療前（P＜0.05）；治療組的發(fā)作頻率、每次持續(xù)時(shí)間、VAS評(píng)分低于對(duì)照組（P＜0.05）。治療后，兩組腦部前動(dòng)脈、中動(dòng)脈、后動(dòng)脈的平均血流速度均顯著降低（P＜0.05）；治療組的腦部前動(dòng)脈、中動(dòng)脈、后動(dòng)脈的平均血流速度低于對(duì)照組（P＜0.05）。治療后，兩組的血清基質(zhì)金屬蛋白-9（MMP-9）水平顯著降低，血清P物質(zhì)（SP）、β-內(nèi)啡肽（β-EP）水平顯著升高（P＜0.05）；治療后，治療組的血清MMP-9水平低于對(duì)照組，血清SP、β-EP水平高于對(duì)照組（P＜0.05）。結(jié)論 全天麻膠囊聯(lián)合尼莫地平片可提高偏頭痛的臨床療效，減輕臨床癥狀，改善腦部血流動(dòng)力學(xué)水平和血清因子水平。

Objective To investigate the clinical effect of Quantianma Capsules combined with Nimodipine Tablets in treatment of migraine. Methods Patients (88 cases) with migraine in Shanghai Songjiang District Sijing Hospital from February 2021 to April 2023 were divided into control and treatment groups according to the random number table method, and each group had 44 cases. Patients in the control group were po administered with Nimodipine Tablets, 2 tablets/time, three times daily. Patients in the treatment group were po administered with Quantianma Capsules on the basis of the control group, 6 grains/time, three times daily. Patients in two groups were treated for 12 weeks. After treatment, the clinical efficacies, migraine symptom, cerebral hemodynamic indexes, and serum indexes in two groups were compared. Results After treatment, the total effective rate of the treatment group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the frequency and duration of migraine attacks and VAS scores of two groups were lower than those before treatment (P < 0.05), and the frequency and duration of migraine attacks and the VAS scores of the treatment group were lower than those of the control group (P < 0.05). After treatment, the average blood flow velocity of anterior, middle, and posterior cerebral arteries was significantly decreased in two groups (P < 0.05), and the average blood flow velocity of anterior, middle, and posterior cerebral arteries in the treatment group was lower than those in the control group (P < 0.05). After treatment, the serum levels of MMP-9 in two groups were significantly decreased, but the serum levels of SP and β-EP in two groups were significantly increased (P < 0.05). After treatment, the serum levels of MMP-9 in the treatment group was lower than those in the control group, but the serum levels of SP and β-EP in the treatment group were higher than those in the control group (P < 0.05). Conclusion Quantianma Capsules combined with Nimodipine Tablets can improve the curative effect of migraine, can reduce the clinical effect, improve the level of cerebral hemodynamics, and improve serum indexes.
