目的 探討桉檸蒎腸溶軟膠囊聯(lián)合兩性霉素B治療真菌性鼻竇炎的臨床療效。方法 選取2021年7月—2023年7月青島市城陽區(qū)人民醫(yī)院收治的112例真菌性鼻竇炎患者，隨機分為對照組（56例）和治療組（56例）。對照組患者靜脈滴注兩性霉素B，25 mg加入生理鹽水100 mL，每2天1次。治療組在對照組的基礎(chǔ)上口服桉檸蒎腸溶軟膠囊，0.3 g/次，3次/d。兩組治療14 d。觀察兩組患者臨床療效，比較治療前后兩組患者癥狀緩解時間，生活質(zhì)量量表（SF-36），血清因子干擾素-γ（INF-γ）、樹突狀細胞相關(guān)C型凝集素-1（Dectin-1）、腫瘤壞死因子-α（TNF-α）和白細胞介素-17（IL-17），及不良反應(yīng)情況。結(jié)果 治療后，治療組臨床總有效率為98.21%，明顯高于對照組總有效率（82.14%，P＜0.05）。治療后，治療組癥狀緩解時間均明顯早于對照組（P＜0.05）。治療后，兩組患者內(nèi)容生理功能（RP）、社會功能（SF）、情感職能（RE）、總體健康（CH）評分比治療前均明顯升高（P＜0.05），且治療組評分明顯高于對照組（P＜0.05）。治療后，兩組血清因子IL-17、TNF-α、Dectin-1明顯降低，而INF-γ水平明顯升高（P＜0.05），且治療組血清因子水平明顯好于對照組（P＜0.05）。治療后，對照組不良反應(yīng)發(fā)生率為14.20%，明顯高于治療組（3.57%，P＜0.05）。結(jié)論 桉檸蒎腸溶軟膠囊聯(lián)合兩性霉素B治療真菌性鼻竇炎可較好改善臨床癥狀，降低鼻竇炎癥反應(yīng)狀態(tài)，有效提升生活質(zhì)量。

Objective To explore the treatment effect of Eucalyptol, Limonene and Pinene Enteric Soft Capsules combined with amphotericin B in treatment of fungal sinusitis. Methods Patients (112 cases) with fungal sinusitis in Qingdao Chengyang People’s Hospital from July 2021 to July 2023 were randomly divided into control (56 cases) and treatment (56 cases) group. Patients in the control group were iv administered with Amphotericin B for injection, 25 mg added into normal saline 100 mL, once every 2 d. Patients in the treatment group were po administered with Eucalyptol, Limonene and Pinene Enteric Soft Capsules on the basis of the control group, 0.3 g/time, three times daily. Patients in two groups were treated for 14 d. After treatment, the clinical evaluations were evaluated, the symptom relief time, SF-36 scores, the levels of serological IL-17, TNF-α, Dectin-1 and INF-γ, and adverse reactions in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group was 98.21%, which was significantly higher than that of the control group (82.14%, P < 0.05). After treatment, the time of symptom relief in the treatment group was significantly earlier than that in the control group (P < 0.05). After treatment, the scores of RP, SF, RE and CH in two groups were significantly higher than those before treatment (P < 0.05), and the scores in the treatment group were significantly higher than those in the control group (P < 0.05). After treatment, the levels of serum factors IL-17, TNF-α and Dectin-1 in two groups were significantly decreased, while the level of INF- γ was significantly increased (P < 0.05), and the level of serum factors in the treatment group was significantly better than that in the control group (P < 0.05). After treatment, the incidence of adverse reactions in the control group was 14.20%, which was significantly higher than that in the treatment group (3.57%, P< 0.05). Conclusion Amphotericin B combined with Eucalyptol, Limonene and Pinene Enteric Soft Capsules in treatment of fungal sinusitis can improve clinical symptoms, reduce the state of nasal sinus inflammation and improve the quality of life.
