目的 探討腦安滴丸聯(lián)合丁苯酞氯化鈉注射液治療急性腦梗死的臨床療效。方法 選取2022年1月—12月北京市隆福醫(yī)院收治的146例急性腦梗死患者，按隨機(jī)數(shù)字表法將所有患者分為對(duì)照組和治療組，每組各73例。對(duì)照組靜脈滴注丁苯酞氯化鈉注射液，100 mL/次，2次/d，滴注時(shí)間≥50 min/次。治療組在對(duì)照組基礎(chǔ)上口服腦安滴丸，20粒/次，2次/d。兩組療程14 d。觀察兩組的臨床療效，比較治療前后兩組簡(jiǎn)式Fugl-Meyer評(píng)定量表（FMA）和美國(guó)國(guó)立衛(wèi)生研究院卒中量表（NIHSS）評(píng)分、血栓及血小板參數(shù)及血清C反應(yīng)蛋白（CRP）、一氧化氮（NO）、超氧化物歧化酶（SOD）、谷氨酸（Glu）水平。結(jié)果 治療后，治療組總有效率是95.89%，顯著高于對(duì)照組的86.30%（P＜0.05）。治療后，兩組FMA評(píng)分均較治療前顯著升高，而NIHSS評(píng)分顯著降低（P＜0.05）；治療組FMA、NIHSS評(píng)分改善更顯著（P＜0.05）。治療后，兩組血漿纖溶酶-抗纖溶酶復(fù)合物（PAP）、凝血酶抗凝血酶復(fù)合物（TAT）和血小板聚集率（PAgT）均較治療前顯著下降（P＜0.05）；且以治療組下降更顯著（P＜0.05）。治療后，兩組患者血清CRP、Glu水平均顯著下降，而血清NO、SOD水平均顯著上升（P＜0.05）；且治療組血清CRP、NO、SOD、Glu水平改善更顯著（P＜0.05）。結(jié)論 腦安滴丸聯(lián)合丁苯酞氯化鈉注射液治療急性腦梗死可獲得良好的療效，能安全有效地抗血栓形成，緩解炎癥及氧化應(yīng)激損害，促進(jìn)神經(jīng)及運(yùn)動(dòng)功能改善。

Objective To explore the clinical efficacy of Naoan Dropping Pills combined with butylphthalide in treatment of acute cerebral infarction. Methods A total of 146 patients with acute cerebral infarction admitted to Beijing Longfu Hospital from January to December 2022 were selected and divided into control group and treatment group according to random number table method, with 73 patients in each group. Patients in the control group were iv administered with Butylphthalide and Sodium Chloride Injection, 100 mL/time, twice daily, infusion time ≥ 50 min/time. Patients in the treatment group were po administered with Naoan Dropping Pills on the basis of the control group, 20 pills/time, twice daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacy was evaluated, and the scores of the simplified Fugl Meyer Rating Scale (FMA) and the National Institutes of Health Stroke Scale (NIHSS), the parameters of thrombus and platelet, and the levels of serum C-reactive protein (CRP), nitric oxide (NO), superoxide dismutase (SOD), glutamic acid (Glu) were compared between the two groups before and after treatment. Results After treatment, the total effective rate of the treatment group was 95.89%, which was significantly higher than that of the control group (86.30%, P < 0.05). After treatment, FMA score in both groups was significantly higher than before treatment, but NIHSS score was significantly lower (P < 0.05). FMA and NIHSS scores in the treatment group were significantly improved (P < 0.05). After treatment, plasma plasminase-anti-plasminase complex (PAP), thrombin antithrombin complex (TAT) and platelet aggregation rate (PAgT) in two groups were significantly decreased compared with before treatment (P < 0.05). The decrease was more significant in the treatment group (P < 0.05). After treatment, serum CRP and Glu levels were significantly decreased, while serum NO and SOD levels were significantly increased in two groups (P < 0.05). The serum levels of CRP, NO, SOD and Glu in the treatment group were significantly improved (P < 0.05). Conclusion Naoan Dropping Pills combined with butylphthalide has good therapeutic effect in treatment of acute cerebral infarction, and can safely and effectively anti-thrombosis, relieve inflammation and oxidative stress damage, promote the improvement of nerve and motor function.
