目的 探討通天口服液聯(lián)合鹽酸倍他司汀片治療前庭性偏頭痛的臨床療效。方法 選取2020年1月-2022年12月上海市寶山區(qū)仁和醫(yī)院治療110例前庭性偏頭痛患者,按照隨機(jī)數(shù)字表法將患者分對(duì)照組(55例)和治療組(55例)。對(duì)照組患者口服鹽酸倍他司汀片,2片/次,2次/d。在此基礎(chǔ)上,治療組患者口服通天口服液,第1天于即刻、服藥1、2、4 h時(shí)口服通天口服液10 mL,以后每6 h口服10 mL,之后10 mL/次,3次/d。兩組患者均連續(xù)治療2個(gè)月。比較兩組的臨床療效、椎基底動(dòng)脈血流速度、頭痛程度、眩暈程度、病癥發(fā)作次數(shù)、睡眠質(zhì)量。結(jié)果 治療組的明顯改善率為83.64%,明顯高于對(duì)照組的明顯改善率65.45%(P<0.05)。治療后,兩組左側(cè)大腦椎動(dòng)脈(LVA)、右側(cè)大腦椎動(dòng)脈(RVA)、基底動(dòng)脈(BA)平均血流速度、視覺模擬評(píng)分法(VAS)、眩暈障礙量表(DHI)評(píng)分、病癥每月發(fā)作次數(shù)、匹茲堡睡眠質(zhì)量指數(shù)量表(PSQI)評(píng)分均顯著降低(P<0.05),且治療組的各指標(biāo)比對(duì)照組更低(P<0.05)。結(jié)論 通天口服液聯(lián)合鹽酸倍他司汀片治療前庭性偏頭痛療效較好,可改善椎基底動(dòng)脈狹窄,有效減輕患者頭痛、眩暈程度,對(duì)降低病癥發(fā)作頻率,改善患者睡眠質(zhì)量有積極影響。

Objective To explore the clinical efficacy of Tongtian Oral Solution combined with Betahistine Hydrochloride Tablets in treatment of vestibular migraine. Methods Patients (110 cases) with vestibular migraine in Shanghai Baoshan District Renhe Hospital from January 2020 to December 2022 were divided into control and treatment groups according to the random number table metho, and each group had 55 cases. Patients in the control group were po administered with Betahistine Hydrochloride Tablets, 2 tablets/time, twice daily. Patients in the treatment group were po administered with Tongtian Oral Solution on the basis of the control group, Tongtian Oral Solution was taken orally 10 mL at 0 h, 1 h, 2 h, and 4 h on the first day, then 10 mL every 6 h, then 10 mL/time, three times daily. Patients in two groups were treated for 2 months. The clinical efficacies, the blood flow velocity of vertebrobasilar artery, the degree of headache, the degree of vertigo, the number of monthly episodes of symptoms, and sleep quality in two groups were compared. Results The improvement rate of the treatment group was 83.64%, which was significantly higher than 65.45% of the control group (P < 0.05). After treatment, the average blood flow velocity of LVA, RVA, and BA, the VAS scores and DHI scores, the monthly frequency of symptoms and PSQI score of two groups were significantly decreased (P < 0.05), and the indexes of the treatment group were lower than those of the control group (P < 0.05). Conclusion Tongtian Oral Liquid combined with Betahistine Hydrochloride Tablets has a good effect on the treatment of vestibular migraine, which can improve vertebrobasilar artery stenosis, effectively reduce the degree of headache and vertigo, reduce the frequency of symptoms and improve the sleep quality of patients.
