[摘要]
目的 對比疏血通注射液與低分子肝素鈣(LMWH)用于外科術(shù)后的抗凝效果。方法 選取普通外科手術(shù)病例80例,年齡45~75歲,根據(jù)入院時間先后順序隨機分為兩組,疏血通組(41例)術(shù)后靜滴疏血通注射液抗凝,LMWH組(39例)術(shù)后皮下注射低分子肝素鈣抗凝。通過觀察并記錄患者治療前后凝血酶-抗凝血酶III復合物(TAT)值、血紅蛋白(Hb)、纖維蛋白原(FIB)、血漿凝血酶原時間(PT)、血小板計數(shù)(PLT)及隨訪3個月期間不良反應(yīng)情況,評價疏血通注射液與低分子肝素鈣的外科術(shù)后的抗凝效果。結(jié)果 與術(shù)前相比,術(shù)后1 d時兩組TAT值明顯增加(P<0.05),使用抗凝藥物后,術(shù)后5、7 d時兩組TAT值相比術(shù)后1 d時明顯下降(P<0.05),且術(shù)后5、7 d時LMWH組TAT值明顯低于疏血通組 (P<0.05);術(shù)后1 d與術(shù)后5 d相比,兩組在血紅蛋白水平、血小板計數(shù)上均沒有顯著差異。與術(shù)后1 d相比,術(shù)后5 d兩組FIB值均明顯降低(P<0.05),PT值均明顯延長(P<0.05)。隨訪3個月期間,兩組不良反應(yīng)率相比,差異沒有統(tǒng)計學意義。結(jié)論 低分子肝素鈣和疏血通注射液均具有較好的抗凝作用,低分子肝素鈣抗凝作用稍強,兩藥不良反應(yīng)發(fā)生率無統(tǒng)計學意義,用于外科術(shù)后抗凝,均安全有效。

[Abstract]
Objective To compare the efficacy of Shuxuetong Injection and low molecular weight heparin (LMWH) for anticoagulation after surgery. Methods Totally 80 patients who had received sugery were selected and divided into two groups according to the sequence of admisson time. The Shuxuetong group (41 cases) was iv given Shuxuetong Injection for anticoagulation. The LMWH group (39 cases) was given low molecular weigth heparin calcium by hypodermic injection. The efficacy of anticoagulation of Shuxuetong Injection and low molecular weight heparin calcium after surgery was evaluated by TAT, Hb, FIB, PT, PLT and adverse reaction during 3 months follow-up. Results Compared with the value before surgery, the TAT values were increased after 1 d surgery (P<0.05). After the use of anticoagulant drugs, the TAT values in 5 d and 7 d after surgery were decreased significantly (P<0.05). And the TAT values in the LMWH group were lower than those of the Shuxuetong group. There was no statistical significance on Hb and PLT between two groups. Compare with the value after 1 d surgery, the FIB level was decreased and PT was extended (P<0.05). There were no statistical significance on adverse reaction between two groups during 3 months follow-up. Conclusion Low molecular weight heparin calcium and Shuxuetong Injection both have good anticoagulant effect. Low molecular weight heparin calcium has a reasonable price and stronger anticoagulation. There is no statistical significance on adverse reaction between two groups. Both of them could be used for anticoagulation after surgical operation.
