目的 探討吡格列酮對(duì)2型糖尿病周圍神經(jīng)病變患者血尿酸變化的影響。方法 收集2014年7月-2015年7月在南通市通州區(qū)第三人民醫(yī)院住院并確診患有糖尿病周圍神經(jīng)病變的患者80例，隨機(jī)分為對(duì)照組和觀察組各40例，兩組患者均進(jìn)行飲食控制結(jié)合運(yùn)動(dòng)療法等一般治療方式。對(duì)照組給予二甲雙胍治療，0.5 g/次，每日3次。觀察組在對(duì)照組的基礎(chǔ)上給予吡格列酮治療，吡格列酮?jiǎng)┝繛?5 mg/次，每日1次。兩組服藥時(shí)間均為2個(gè)月。結(jié)果 兩組空腹血糖及餐后2 h血糖濃度經(jīng)治療后均降低，觀察組低于對(duì)照組（P<0.05）；對(duì)照組治療前后血尿酸差異無明顯變化（P>0.05），觀察組治療后血尿酸濃度顯著降低（P<0.05）；兩組治療后CSS評(píng)分均低于治療前評(píng)分（P<0.05）；治療后觀察組評(píng)分低于對(duì)照組（P<0.05）。結(jié)論 吡格列酮聯(lián)合二甲雙胍對(duì)治療2型糖尿病周圍神經(jīng)病變有一定療效，可降低2型糖尿病周圍神經(jīng)病變患者血尿酸水平。

Objective To investigate the correlation of pioglitazone on serum uric acid in patients with type 2 diabetic peripheral neuropathy. Methods Totally 80 cases of patients diagnosed with type 2 diabetic peripheral neuropathy from July 2014 to July 2015 in the Third People's Hospital of Tongzhou were collected and randomly and averagely divided into control group and observation group. The two groups were both given diet intervention and sports therapy. Then the patients in control group were given Metformin treatment, 0.5 g every time, three times daily. The patients in observation group were given the same treatment as in control group and meanwhile given pioglitazone treatment, 15 mg every time, once daily. Both groups were given treatment for 2 months. Results The fasting blood glucose and postprandial 2 h blood glucose concentration were lower after treatment in both groups, and the level in observation was lower than that in the control group, (P<0.05); The changes of blood uric acid in the control group showed no obvious difference after treatment (P > 0.05),and the changes in observation group decreased after treatment (P<0.05); After treatment, the CSS grade in two groups was lower than before (P<0.05), and the score in observation group was lower than that in control group (P<0.05). Conclusion Pioglitazone combined with metformin shows certain curative effect for type 2 diabetic peripheral neuropathy and can reduce uric acid level in patients with type 2 diabetic peripheral neuropathy.
