目的 研究參附注射液聯(lián)合小劑量糖皮質(zhì)激素治療感染性休克的臨床效果。方法 選擇2013年1月-2017年1月在新疆阿克蘇第一人民醫(yī)院進(jìn)行診治的感染性休克患者94例，隨機(jī)分為兩組，每組各47例。對照組給予小劑量糖皮質(zhì)激素治療，觀察組在對照組基礎(chǔ)上給予參附注射液治療，兩組均持續(xù)治療2 d。比較兩組的臨床治療效果，治療前、治療后1 d和治療后2 d的白介素-6、腫瘤壞死因子-α以及血液乳酸水平；觀察兩組的住院時間，并隨訪半年，記錄兩組患者的多臟器功能不全發(fā)生率以及病死率。結(jié)果 觀察組的有效率為91.49%（43/47），明顯高于對照組的76.59%（36/47），差異有統(tǒng)計學(xué)意義（P<0.05）。兩組治療后1 d和治療后2 d的白介素-6和腫瘤壞死因子-α水平均明顯降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05），且觀察組明顯低于對照組，差異有統(tǒng)計學(xué)意義（P<0.05）。兩組治療后1 d和治療后2 d的血乳酸水平均明顯降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且觀察組明顯低于對照組，差異有統(tǒng)計學(xué)意義（P<0.05）。觀察組的住院時間以及多臟器功能不全發(fā)生率明顯低于對照組，差異有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 參附注射液聯(lián)合小劑量糖皮質(zhì)激素治療感染性休克的臨床效果明顯優(yōu)于單純采用西藥治療，能有效改善患者的血乳酸和炎癥因子水平，并具有較高的安全性，適合推廣應(yīng)用。

Objective To investigate the clinical effect of Shenfu Injection combined with small dose of glucocorticoid in the treatment of septic shock.Methods Selected 94 cases of patients with septic shock who were treated in our hospital from January 2013 to January 2017, divided into two groups randomly. The control group was treated with small dose of glucocorticoid, and the observation group was treated with Shenfu Injection. The clinical therapeutic effect of two groups were compared, interleukin -6, tumor necrosis factor alpha and blood lactate levels before and after treatment were compared, and followed up for half a year, recorded multiple organ function of the two groups of patients the incidence and mortality rate.Results After treatment, the effective rate of the observation group was 91.49% (43/47), significantly higher than that of the control group 76.59% (36/47) (P < 0.05); the levels of interleukin -6 and tumor necrosis factor alpha after treatment were significantly lower in the two groups (P < 0.05) than in the observation group (P < 0.05); the blood lactate level were significantly decreased after treatment (P < 0.05), and the observation group was more significant (P < 0.05); hospitalization time of the observation group and multi organ dysfunction was significantly lower than the control group (P < 0.05).Conclusion Shenfu Injection combined with small dose of glucocorticoid in the treatment of septic shock is better than the simple use of western medicine treatment, can effectively improve the patient's blood lactic acid and the levels of inflammatory factors, and has high security.
