目的 探討小金膠囊聯(lián)合注射用醋酸亮丙瑞林微球治療卵巢巧克力囊腫的臨床效果。方法 選取2015年5月—2017年1月咸陽(yáng)市第一人民醫(yī)院收治的116例卵巢巧克力囊腫腹腔鏡手術(shù)后患者作為研究對(duì)象，前瞻性隨機(jī)分為治療組及對(duì)照組，每組58例。對(duì)照組在術(shù)后首次月經(jīng)來潮第1天皮下注射給予注射用醋酸亮丙瑞林微球。治療組則在對(duì)照組的基礎(chǔ)上另口服給予小金膠囊，5粒/次，2次/d。兩組均連續(xù)治療6個(gè)月，并隨訪1年。觀察兩組的臨床療效，比較兩組性激素指標(biāo)［雌激素（E2）、卵泡刺激素（FSH）及黃體生成素（LH）］，子宮動(dòng)脈血流參數(shù)［搏動(dòng)指數(shù)（PI）、收縮期峰值流速/舒張末期流速比值（S/D）及阻力指數(shù)（RI）］，卵巢巧克力囊腫標(biāo)記物水平［抗子宮內(nèi)膜抗體（EMAb）、基質(zhì)金屬蛋白酶-3（MMP-3），前列腺素F2α（PGF2α）及核因子κB（NF-κB）］，生活質(zhì)量指標(biāo)（生理領(lǐng)域、環(huán)境領(lǐng)域、社會(huì)關(guān)系領(lǐng)域及生活領(lǐng)域評(píng)分）和1年復(fù)發(fā)率。結(jié)果 治療后，治療組總有效率明顯高于對(duì)照組（P<0.05）。治療后，兩組性激素指標(biāo)（E2、FSH及LH），子宮動(dòng)脈血流參數(shù)（PI、S/D及RI），卵巢巧克力囊腫標(biāo)記物水平（EMAb、MMP-3，PGF2α及NF-κB），生活質(zhì)量指標(biāo)（生理領(lǐng)域、環(huán)境領(lǐng)域、社會(huì)關(guān)系領(lǐng)域及生活領(lǐng)域評(píng)分）均顯著得到改善，且治療組上述指標(biāo)的改善程度均明顯優(yōu)于對(duì)照組（P<0.05）。隨訪1年，治療組復(fù)發(fā)率明顯低于對(duì)照組（P<0.05）。結(jié)論 小金膠囊聯(lián)合注射用醋酸亮丙瑞林微球治療卵巢巧克力囊腫具有較好的臨床療效，值得進(jìn)行深入研究。

Objective To investigate the clinical effect of Xiaojin Capsules combined with Leuprorelin Acetate Microspheres for Injection in treatment of ovarian chocolate cyst. Methods 116 patients with ovarian chocolate cyst in Xianyang First People's Hospital from May 2015 to January. 2017 were randomly divided into control and treatment groups, 58 cases in each group. Patients in the control group were treated with Leuprorelin Acetate Microspheres for Injection on day 1 of first postoperative menstruation. Patients in the treatment group were given Xiaojin Capsules after the operation on the basis of the control group, 5 grains/time, 3 times/day, for 6 months, and were followed up for 1 year. After treatment, the clinical efficacy was evaluated, and the changes of sex hormone indicators[estrogen 2 (E₂), follicle-stimulating hormone (FSH) and luteinizing hormone (LH)], uterine artery blood flow parameters[pulsation index (PI) and systolic/diastolic peak flow velocity at the end of the velocity ratio (S/D) and resistance index (RI)], levels of ovarian chocolate cyst markers[endometrial antibody (EMAb), matrix metalloproteinases-3 (MMP-3), prostaglandin F2 alpha (PGF2 alpha) and nuclear factor kappa B (nf-kappa B)] predominate, quality of life index (physical field, environment, social relationship and life grade) and 1 year recurrence rate were compared. Results After treatment, the clinical efficacy in the treatment group was significantly higher than the control group (P<0.05). After treatment, the levels of sex hormone indicators[estrogen 2 (E₂), follicle-stimulating hormone (FSH) and luteinizing hormone (LH)], uterine artery blood flow parameters[pulsation index (PI) and systolic/diastolic peak flow velocity at the end of the velocity ratio (S/D) and resistance index (RI)], levels of ovarian chocolate cyst markers[endometrial antibody (EMAb), matrix metalloproteinases-3 (MMP-3), prostaglandin F2 alpha (PGF2 alpha) and nuclear factor kappa B (nf-kappa B)] predominate, quality of life index (physical field, environment, social relationship and life grade) in the 2 groups were significantly promoted (P<0.05), while the treatment group were significantly improved than those in the control group (P<0.05). Follow up for 1 year, the recurrence rates in the treatment group was significantly lower than the control group (P<0.05).Conclusion Xiaojin Capsules combined with Leuprorelin Acetate Microspheres for Injection in treatment of ovarian chocolate cyst has significant clinical effect in treatment of ovarian chocolate cyst, which has a certain clinical application value.
