[摘要]
目的 探討雙歧桿菌四聯(lián)活菌片聯(lián)合多潘立酮對小兒功能性消化不良胃電節(jié)律、血漿P物質及胃動素水平的影響。方法 選取2016年3月—2019年3月徐州市第一人民醫(yī)院108例功能性消化不良患兒作為研究對象,根據(jù)隨機數(shù)字表法將患者分為對照組和觀察組,每組各54例。對照組患兒餐前30 min口服多潘立酮混懸液,3歲以內3~4 mL/次,3次/d;4~6歲患兒5~6 mL/次,3次/d;7歲以上患兒7~10 mL/次,3次/d。觀察組患兒在對照組的基礎上口服雙歧桿菌四聯(lián)活菌片,1~6歲患兒1.0 g/次,2~3次/d;6歲以上患兒1.0~1.5 g/次,3次/d。兩組均連續(xù)治療14 d。觀察兩組患兒的臨床療效,同時比較兩組治療前后的癥狀評分、胃電節(jié)律異常發(fā)生率、血漿P物質和胃動素水平。結果 治療后,觀察組患兒治療總有效率為94.44%,顯著高于對照組的79.63%,兩組比較差異具有統(tǒng)計學意義(P<0.05)。治療后,兩組患者各癥狀評分均明顯降低(P<0.05),且觀察組癥狀評分顯著低于對照組(P<0.05)。治療后,兩組患兒餐前及餐后胃電節(jié)律異常發(fā)生率均明顯降低(P<0.05),且觀察組胃電節(jié)律異常發(fā)生率顯著低于對照組(P<0.05)。治療后,兩組患兒血漿P物質及胃動素水平均明顯升高(P<0.05),且觀察組P物質及胃動素水平顯著高于對照組(P<0.05)。結論 雙歧桿菌四聯(lián)活菌片聯(lián)合多潘立酮可通過改善胃電節(jié)律、提高血漿P物質及胃動素水平的影響來改善患兒臨床癥狀,療效確切,值得在小兒功能性消化不良治療中應用推廣。

Objective To investigate the effects of Combined Bifidobacterium, Lactobacillus, Enterococcus and Bacillus combined with domperidone on gastric electric rhythm, plasma substance P, and motilin levels in children with functional dyspepsia. Methods A total of 108 children with functional dyspepsia in Xuzhou NO.1 People's Hospital from March 2016 to March 2019 were selected as the research objects. According to the random number table method, the patients were divided into the control group and the observation group, with 54 patients in each group. Children in the control group were po administered with Domperidone Suspension at 30 min before meal, 3-4 mL/time, and three times daily for children within 3 years old. 5-6 mL/time, three times daily for children aged 4-6 years, and 7-10 mL/time, three times daily for children over 7 years old. Children in the observation group were po administered with Combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus on the basis of control group, children aged 1-6 years were given 1.0 g/time, 2-3 times/daily. Children over 6 years of age, 1.0-1.5 g/time, three times daily. Both groups received continuous treatment for 14 days. After treatment, the clinical efficacy in two groups of children was observed, and the symptom scores, incidence of abnormal gastric electric rhythm, plasma substance P, and motilin levels of two groups before and after treatment were compared. Results After treatment, the total effective rate of the observation group was 94.44%, which was significantly higher than 79.63% of the control group, and the difference between two groups was statistically significant (P<0.05). After treatment, the symptom scores in two groups were significantly decreased (P<0.05), and the symptom scores of the observation group were significantly lower than those of the control group (P<0.05). After treatment, the incidence of abnormal gastric electric rhythm in two groups was significantly decreased (P<0.05), and the incidence of abnormal gastric electric rhythm in the observation group was significantly lower than that in the control group (P<0.05). After treatment, the plasma levels of substance P and motilin in two groups were significantly increased (P<0.05), and the levels of substance P and motilin in the observation group were significantly higher than those in the control group (P<0.05). Conclusion Combined Bifidobacterium, Lactobacillus, Enterococcus and Bacillus combined with domperidone can improve the clinical symptoms of children by improving gastric electric rhythm and increasing the influence of substance P in plasma and motilin level. It has a definite effect and is worthy of application and promotion in treatment of functional dyspepsia in children.
