[摘要]
目的 探討腎衰寧顆粒聯(lián)合連續(xù)性血液透析對腎衰患者炎癥反應、殘余腎功能及結(jié)締組織生長因子(CTGF)表達的影響。方法 選取2022年7月—2023年7月在上海交通大學醫(yī)學院附屬第九人民醫(yī)院黃浦分院就診的200例腎衰患者,隨機將其分為對照組和試驗組,每組各100例,兩組患者均進行連續(xù)性血液透析治療,試驗組在此基礎上給予腎衰寧顆粒治療,每次 1 袋,每天 3 次,治療 3 個月。觀察兩組炎癥反應、殘余腎功能及 CTGF 表達等情況。結(jié)果 試驗組總有效率(94.00%)明顯高于對照組(71.00%),組間比較差異顯著(P<0.05)。治療前兩組白細胞介素-6(IL-6)、腫瘤壞死因子-α(TNF-α)、核因子 κB 抑制蛋白-α(IκB-α)水平對比無明顯差異;治療后,兩組 IL-6、TNF-α、IκB-α 水平均顯著降低(P<0.05),且試驗組比對照組降低明顯(P<0.05)。治療前兩組血肌酐(SCr)、血胱抑素-C(CCys-C)、腎小球濾過率(GFR)水平對比無明顯差異;治療后,兩組 SCr、CCys-C、GFR水平均顯著降低(P<0.05),且試驗組明顯低于對照組(P<0.05)。治療前兩組CTGF表達對比無明顯差異;治療后,兩組CTGF表達顯著降低(P<0.05),且試驗組比對照組降低明顯(P<0.05)。試驗組總不良反應率(5.00%)明顯低于對照組(13.00%),組間差異有統(tǒng)計學意義(P<0.05)。結(jié)論 腎衰寧顆粒聯(lián)合連續(xù)性血液透析對腎衰患者具有顯著療效,其可顯著改善炎癥反應,提高殘余腎功能,并顯著抑制CTGF表達。

Objective To explore the study of continuous hemodialysis combined with Shenshuaining Granule on inflammatory response, residual renal function and connective tissue growth factor (CTGF) expression in renal failure patients. Methods From July 2022 to July 2023, 200 patients with renal failure who attended the Huangpu branch of the Ninth People's Hospital affiliated to Shanghai Jiaotong University School of Medicine were selected and randomly divided into control group and experimental group, 100 cases in each group, and both groups were treated with continuous hemodialysis, and the experimental group was given Shenshuaining Granule on the basis of which, one sachet per time, three times per day, treatment for three months. Observe the inflammatory reaction, residual renal function and CTGF expression in both groups. Results Compared with the control group, the total effective rate of the experimental group (94.00%) was significantly higher than that of the control group (71.00%), and the difference between the groups was obvious (P <0.05). There was no significant difference in the levels of interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α), and nuclear factor κB inhibitory protein-α (IκB-α) between the two groups before treatment; after treatment, the levels of IL-6, TNF-α, and IκB-α in the two groups were significantly reduced (P <0.05), and the experimental group was reduced significantly compared with the control group (P <0.05). There was no significant difference in the comparison of blood creatinine (SCr), blood cystatin-C (CCys-C), and glomerular filtration rate (GFR) levels between the two groups before treatment; after treatment, the levels of SCr, CCys-C, and GFR in the two groups were significantly reduced (P <0.05), and the experimental group was significantly lower than the control group (P <0.05). There was no significant difference in the comparison of CTGF expression between the two groups before treatment; after treatment, CTGF expression was significantly reduced in both groups (P <0.05), and the test group was significantly lower than the control group (P <0.05). The total adverse reaction rate of the experimental group (5.00%) was significantly lower than that of the control group (13.00%), and the difference between the groups was statistically significant (P <0.05). Conclusion Continuous hemodialysis combined with Shenshuaining Granule has significant efficacy in patients with renal failure, which can significantly improve the inflammatory response, improve residual renal function, and significantly inhibit CTGF expression.
